Application Form to join Creative Age at Chelteham Quakers
Creative Age is for people living with dementia at home and their supporters/family/carers.
To apply to to join our sessions you can download an application form or complete the online application below.

Once completed please you can send your form via email, and send to openarmsartists@gmail.com
After your application has been received, one of the Creative Age team will phone you. We will then arrange a home visit so you can meet us, and you can then find out more about what to expect at Creative Age.
